Employer Request for Background Check - Updated 2010 - Positions that are paid will need to fill out this form for their employer - this form would be suited to coaches or other paid positions within your club (please check with your employer). Please send this form and Page 1 of the Applicant Declaration and Consent form for each person to be checked to your Approved Screening Agency. You may also submit these details using your Approved Screening Agency's on-line processes. BE SURE to keep this forms for your club/individual records.
Applicant Declaration and Consent Form - Updated 2010 - Volunteer positions will need to fill out this form for their club - this form would be suited to technical officials, club comittee members and any other member who works with or is associated with children in your club. You will need to sign the printed form and provide the signed form to your prospective employer/volunteer organisation.
